Simple bronze marker affixed to a white granite base, located in the Heavenly Rest Catholic Cemetery, near the cremation wall.

The marker was denoted in 1990 by the Royal Canadian Legion Branch 2in Windsor and Unit 126 of the Polish Army Veterans Association.

Text (in English and Polish) reads:

FOR YOURS AND OUR FREEDOM

In Memory Of The Veterans
Who Fought In The Allied Polish Armed Forces
In The Battles Of Land, Sea and Air

The marker is framed by two flag poles. One carries the Canadian flag, the other the Polish flag.
