A zinc headstone contains a draped obelisk topped by an urn and a flame. It is the poignant grave marker which contains the names, dates of death and ages for four young children. The base on the front of the marker is inscribed O.W. KELLOGG, M.D..
The panel on the front of the monument has a Latin cross wrapped in roses. The left panel is inscribed:
