I was told this is how it is done, A frame of wood is constructed at the grave site, concrete made and pored, then smoothed (or in concrete terms -finished), then topped (pressed) with a plaster raised letter mold to imprint the text.
To created the mold pressed in the concrete. A smooth pine board is engraved with the letters using a LeRoy set (woodworkers know what it is), this is a negative mold, the it is covered with plaster and allowed to set. This creates the positive (or raised letter) mold.
Sounds like a lot of work, and it is, but the cost is just a couple of dollars even in today's (2017) market.
There are two like this in the cemetery, I had never seen it before, and they creation of each is almost 40 years apart. And they are side-by-side in the cemetery
