The memorial monument is a simple red granite marker, polished front and back with a rough hewn edge. It is incised with the same inscription on both sides in gold lettering and below this is a shallow relief of the Staffordshire Police Authority emblem.
The inscription in roundel reads;

23rd NOVEMBER 1986

The John Taylor Memorial Committee was set up following the death of PC John Taylor who died on duty on 23 November 1986. They award a serving police officer or member of the Special Constabulary who has made an outstanding contribution to the community, performed an act of bravery, or a sporting achievement.
