His grave is in a family plot & is difficult but not impossible to read, being stained with age. Strangely when photographed for the Australian Cemeteries Index in 2005 it was much clearer.

The Headstone is worded:

In loving memory of our dear son & our dear brother Pte Thomas J Wark NX56139 8th Div. 2/19 Batt. Died 12-9-44 aged 34 years. Lost on a prisoner of war ship that was torpedoed at sea.

The Australian War Memorial shows that he died at sea on the Rakuyo Maru. It states in part "On 12 September 1944, two Japanese ships transporting Australian and British prisoners of war from Singapore to Japan were sunk, resulting in the loss of 1,559 Australian and British lives. The convoy was attacked by American submarines in the South China Sea. Rakuyo Maru was sunk by USS Sealion II".
