On a hillside, near the entrance to Graham Memorial Cemetery, this memorial consists of a granite monument, surrounded by a concrete walk. Two flagpoles fly the flags of the United States, and Oklahoma. The memorial has two concrete staircases on the south, leading up from lower ground.

The monument has a flag and eagle engraved at the top and reads: "In memory of our comrades of Mayes Conty, who made the supreme sacrifice for God and Country and were never returned to their native soil / Sullivan-Lewis Post 182." Beneath this are listed the names of thirty-four veterans with their ranks.
