The headstone is made of concrete and is an upright rectangle, but it has a decorative arched design on its top. In the arched area at the top of the headstone is what appears to be a rough etching of the Great Seal of the United States. There is an etching of a star in a box on either side of the eagle. The headstone inscription reads:

During the year there were two epidemics of influenza one from about March 2 to 23 and another from about September 15 to November 15 The epidemic of March was mild in character and not general among the troops on Governors Island confined mainly to the troops or the casual camp. The epidemic of September, October and November was widespread attacking all organizations. Throughout the latter four months of the year a mild epidemic of mumps was present among the troops of the 424th Reserve Labor Battalion Quartermaster Corps.
