This is one of the older Woodmen of the World monuments, and like many in the cemetery, it has not weathered very well in some spots. It's difficult to tell whether the mallet handle is actually broken, or whether that was part of the design. Thanks to Findagrave for providing information about his name, and they indicate that he was buried in 1933 (http://www.findagrave.com/cgi-bin/fg.cgi?page=gr&GRid=35158082). There is no emblem, but the it's not difficult to piece together the inscription about this being a Woodmen of the World:
