The text is in Gold leaf on a black board, all set within a Marble frame. The memorial is displayed within St Mary the Virgin Church.
I am unable to find the date of Dedication, or who sponsored the memorial, but I suspect public subscription of Church parishioners.
The inscription on the plaque reads;
