Located on the side of a building that now serves as the Pier 424 Seafood Market restaurant.
The plaque reads:

"JACK TEAGARDEN

PLAYED HIS LAST STAND HERE
ONE YEAR AGO TODAY
ON 14 JANUARY 1964
THIS PLAQUE IS PLACED HERE IN THIS SPOT
BY YFMNOLA IN DEEPEST RESPECT FOR
HIS ENDURING CONTRIBUTION TO JAZZ"
