The memorial is a rough cut stone with an arched top. Mounted to the stone is a rectangular plaque adorned with an eagle with spread wings. The inscription is "Honor Roll World War Portmouth Rhode Island" The memorial lists about 90 men who served in WWI from Portsmouth.
