The original memorial was erected in Green Park in Heckmondike to honour the memory the men of Heckmondwike who gave their lives in the First World War.
The memorial is in the form of an octagonal base surmounted by plinth and blomfield-style cross. It is inscribed in black lettering on the sides of the plinth and on the base and a bronze sword of sacrifice on the front face.
Since its original creation it has been modified to add inscriptions to commemorate subsequent wars.
On of the modifications is the addition of this plaque of the Burma Star Assocaiation. It has been attached to the third tier of the memorial. At the top of the plaque is an engraving of the Burma Star War medal.

The remaing text on the plaque includes an inscription that contains the epitaph that was originally used in the Kohima Allied war cemetery.
Sadly the text on the plaque has become rather faded but has the following text.
BURMA STAR ASSOCIATION
WHEN YOU GO HOME TELL THEM OF US AND SAY
FOR YOUR TOMORROW WE GAVE OUR TODAY
THE KOHIMA EPITAPH
