Found at the site of the former Mayfield Roller Mills on Cedar Creek in Mayfield, WI. The marker is affixed to the top a concrete block, approximately 4ft tall, on the north side of the creek, at the base of a large oak tree. The block is 40-50ft from the road.

The marker does not bear any identifying numbers. The marker is documented in a January 1969 report, where it is referred to as ML-D1A.
