The official origin of the Cowichan River is the weir at the easternmost end of Cowichan Lake. Floodgates closer to the southern bank allow for the level of the lake to be controlled and help maintain specified summer and early fall flow rates down the river. A boat lock at the northern bank allows boats to bypass the weir. A boat launch and parking area are located beside the boat lock and weir and are accessible from North Shore Road. The coordinates were taken at the closest point to the weir on the northern bank beside this parking area.

After leaving Cowichan Lake, the river flows for 47 kilometres to the east until it empties into Cowichan Bay. The river has been very important to the First Nations peoples of the Cowichan Valley for thousands of years as a source of food, and is still critical to the local economy to this day. The river was designated a Canadian Heritage River in 2003 (WMHV7A) and is considered one of the finest trout fishing rivers in the province. Approximately 30 kilometres of the river flows through Cowichan River Provincial Park (WMCW79). The Koksilah River is the only major tributary and enters the Cowichan River just before its end at Cowichan Bay.
