This aircraft was ordered by the US Air Force as serial number 53-4396, but was delivered to the Royal Canadian Air Force (RCAF) as tail number 9641 on January 12, 1956. Early in its career, it participated in the construction of the Mid-Canada Line (MCL), a predecessor of the Distant Early Warning (DEW) line. In 1958, civilian contractor Spartan Air Services was awarded the contract to provide support to the MCL stations. The helicopter, along with eight sisters, was converted for civilian use, redesignated a model 42A, and given the civilian serial number CF-JJQ. Dominion Helicopters took over the contract and operation of the helicopter in 1959 and continued to operate it until the line was shut down in 1965. The aircraft returned to service with the RCAF in September 1965 as a redesignated CH-125 with its old tail number of 9641 and served until August 1971, when it was struck off and sold to civilian service as serial number CF-GQS. It moved to the Comox Air Force Museum at some unknown later date and was restored to the paint scheme it would have had late in its RCAF career.
