The memorial to Dennis Percival, Eric Tolles, Tom Ballantyne, and recognition of the Nelson Lions Club itself, is in Lakeside Park in the City of Nelson, near the eastern edge of the park.

This small plaque, dedicated to the memory of Lions members Dennis Percival, Eric Tolles and Tom Ballantyne, who have passed on, is mounted on a small stone cairn which is displayed alongside a walkway near the lake. The plaque was placed by the Lions Club of Nelson.
The Lions Club of Nelson was established 1945 and for 80 years have provided community and humanitarian services.
