This mark is a U.S. Coast & Geodetic Survey azimuth mark, located in Redding, CT, in the village of Redding Ridge. It is set in a concrete post on the south side of Church Hill Road. Although the NGS PID is CB4258, the datasheet is somewhat useless; all the information you need is in the datasheet for the primary station, LX3811 (REDDING 1942):

THE AZIMUTH MARK IS A STANDARD NGS DISK
STAMPED---REDDING 1942---,
SET INTO THE TOP OF A SQUARE CONCRETE MONUMENT 30 CM ON A SIDE
PROJECTING 20 CM ABOVE THE GROUND. IT IS LOCATED
4.7 METERS (15.5 FT) SOUTH FROM THE APPROXIMATE CENTER OF CHURCH
HILL ROAD AND
1.5 METERS (5 FT) NORTHWEST FROM THE NORTHWEST CORNER OF A STONE
WALL FENCE CORNER.

TO REACH AZIMUTH MARK FROM THE STATION PROCEED SOUTH ON
STATE HIGHWAY 58 FOR 0.2 KM (0.15 MI) TO A CROSSROAD.
TURN LEFT AND GO EAST ON CHURCH HILL ROAD FOR 0.5 KM
(0.3 MI) TO THE MARK ON RIGHT.

The mark is easy to locate, it is on the east side of a driveway that is blocked by a large wooden gate between large stone pillars and rock walls.

The concrete post covered the outer title ring of the disk, but the disk is definitely an azimuth.

CB4258 is not shown on the Geocaching site, but LX3811 is, and I made a log note at LX3811 that the azimuth was found.

Also note that LX3811 is buried 10 inches beneath the soil in private land, and recovery of that mark is best left to the professionals.
