This fallout shelter sign is located next to a basement entrance at the northwest corner of McKenzie Hall, also known as Ward 40, on the north campus of the Oregon State Hospital. This is one of several fallout shelter signs that can be found at the hospital. This building, erected in 1948, now stands vacant.
