This disk installed on the New Hampshire side of the bridge is actually on New Hampshire ground, by the famous Cornish-Windsor covered bridge over the Connecticut River.

In Cornish, NH, near the entrance to the Cornish-Windsor covered bridge, is a benchmark disk set in concrete near ground level.

The disk is located behind the guard rail at the corner of Bridge Street and NH Route 12A on the New Hampshire (east) side of the Connecticut River. From the bridge, turn right at the intersection and park at a turnout with the historic sign about 100 ft to the south. The road is pretty busy, so be careful. Also, the grass behind the guard rail is inundated with poison ivy - so again, be careful. I'm not overly sensitive to it, myself, so I risked it and looked for areas to step in.

The monument is about 5 ft from the guard rail facing Route 12A and about 10 ft from the guard rail facing Bridge Street. There is an orange colored witness post by the nearest guard rail post, which is how I spotted the disk.

The disk is set in in a concrete base at approximately ground level. The text is as follows. Inside the edge is "Vermont - Geodetic Survey. Inside of this is "For Info Call Agency of Trans." Inside of that is "Cornish Bridge - 2001." At the center is a circle with a dot.

I could not find any specific information on the NGS/Geocaching or any Vermont government sites.

The disk is in good condition.
