The memorial is a bronze tablet mounted on fence in front of what is now known as the Zions Reformed Church of Christ.  The tablet is inscribed:

Of The Soldiers Of The 
Continental Army
Who Suffered And Died In Zion Church
Used As A Military Hospital
From
September 1777 To April 1778.
This Tablet Is Erected
