On 05th July 1978, several minutes after take-off from Lajes Air Force Base the Portuguese Air Force military aircraft model CASA C-212 Aviocar 100 with tail number 6518, flew into the Serra de Santa Bárbara Mountain in Terceira island, Açores, probably due to human error.
There were only three persons on board, all crew members, and they died in this crash, their names are engraved in the plaque on this memorial.
The aircraft was damaged beyond repair.
