Agawam World War I Memorial is in two parts. On an octagonal brick structure, topped with a cupola and a bell, there is a dedication plaque honoring all Agawam veterans and especially those whose names are listed on a plaque elsewhere on the octagonal structure that died in war. On a separate nearby plaque there is a listing of all Agawam citizens who served during United States Wars.
The dedication plaque is inscribed:
