This memorial is a plaque mounted to a large boulder. The memorial sites just outside of the covered picnic shelter and is situated in a nice flower garden.

The plaque honors Norman Hodgson Baker, MD for his humanitarian efforts in creating Maplewood State Park. It also features a carved bust of Dr. Baker.

It reads:

This marker placed so that all generations may know
Norman Hodgson Baker MD
1905-1972
An unexcelled humanitarian and conservationist whose energy, foresight and dedication helped create Maplewood State Park.
May others follow his example.
This memorial dedicated by the Maplewood State Park Association 1973.
