The memorial was dedicated in 1897. The memorial features a sculpture by W. Granville Hastings of a woman (Liberty) wearing a long robe, in her right hand she hold a pike high in the air and in her left she holds a sword. In front of her is figure of a farmer (Patriot) genuflecting as he reaches for the sword. To his left is his plow. The figures stand on a base with wing on each side. On each side are bas relieves of a female reclining figures, one is writing on a tablet and the other is holding a fern. On the center part of the base is a bas relief scene of Union artillery. On the back side of the base is plaque inscribed:

In Grateful Recognition
Of The Valiant And Self-Sacrificing Service To Their Country
Of All Persons
Who Went From Pawtucket And It Vicinity To Join
The Forces Of The United States
During The Civil War.
And Especially To The Memory Of Those Who Suffered
And Died In That Service.
This Monument Is ErectedBy
The Ladies Soldiers Memorial Association
Of Pawtucket, Rhode Island
And By Then Is Consecrated As An Everlasting Memorial
