This survey disk is set into the surface of the southern bridge abuttment of a bridge on Hacks Road just south of Springville Sardinia Highway (Route 39). It can be found on the western side of the abuttment, west of the guardrail.

The bridge seems to have been rebuilt in 1990, as both this elevation marker and a survey disk ("BUCK" on the opposite - or northeastern - corner of the bridge) are both stamped "reset 1990".
