This sculpture is located in Warm Springs Park, northwest of downtown Salt Lake City. It is an older park with many mature trees and large grassy areas. The park runs between Beck Street and Victory Road, from 800 North to about 1000 North.
The Art Inventory lists the description as:
A dog, laying on his side, with all four legs outstretched and head raised up. The dimensions are approximately 14 x 35 x 26 inches.
That pretty much sums it up. It is situated in a grassy area of the park, with no real base to it, so from a distance it looks like an actual dog laying in the park.
