This milestone is located on the north side of Forge Village Road (MA Rt. 225), just east from the junction of Boston Road (MA Rt. 119).

It is a post of granite stone, bearing the inscription 'TO GROTON 4 M. 1903', clearly stating to travelers that they are 4 miles (southeast) from Groton town center.

It is immediately northwest of a junction of Forge Village Road with Fletcher Hill Lane. It is also immediately west from a 'NO PASSING' sign pole.

Historic topographic maps show Forge Village Road as MA State Rt. 25, the previous designator for this road.
