The memorial is at the base of a flag pole in front of Sutter hospital in downtown Sacramento. The plaque reads:
Dedicated this date August 20, 1991. In memory of Ssgt Linda S Simonds. April 8, 1957 - May 13, 1991."
Simonds was in a Army Bell UH-1H helicopter when it crashed during a medical evacuation near Sota Cano Air Base near Comayagua, Honduras. She was part of the 126th Medical Company Air Ambulance of the California National Guard headquartered in Sacramento.
