The plaque reads: "This theater remained a brilliant center of drama until August 11, 1888 - among artists who played here were - Charles W. Couldcock, Edwin Booth - John Broughan - Edwin Adams - Barton Hill - Walter Montgomery - Mrs. D. P. Bowers - Adelaide Neilson - Lotta Crabtree/ Tablet placed by the Commonwealth Club of California and the Pacific Telephone & Telegraph Co. 1932"
At the bottom right corner is the relief of three men playing a flute-like instrument. At the top right corner is a man on one knee playing a banjo to a woman. The whole plaque is outlined with a border.
