This granite post is located on the border of Winchendon and Ashburnham, MA. The google translates the coordinates as "392-398 Sherbert Road Extension", which is the name for the road on the Ashburnham end; Approaching from the Winchendon side this is "North Ashburnham Road". The unusual aspect of this marker is the old signpost that accompanies it; it is 10 feet tall, sturdily built of concrete and steel, and has 3 separate sign plates facing E, W and S, but whatever lettering it carried has long since rusted away.

I would presume the names of the two towns would have been shown here. To find the marker, take Sherbert Road extension, the marker will be visible in the woods, on the right within 50 feet after the pavement ends. DO NOT travel this road beyond this point (or attempt to come in from the other direction) without a 'real' 4WD vehicle.
