This disk is in the MHD database with point ID 12683. The to-reach reads:

"ABOUT 0.4 MI EAST OF WELLINGTON CIRCLE, AT THE MEDFORD-EVERETT TOWN LINE, ON THE ROUTE 16 (MYSTIC VALLEY PARKWAY) BRIDGE OVER THE MALDEN RIVER, AT THE SOUTHWEST CORNER OF THE BRIDGE, 0.5 FT EAST OF WEST END OF SIDEWALK, 4.0 FT SOUTH OF EDGE OF ROAD."
