The plaque is located directly in front of the Randolph First Congregational Church and is in a grassy area between two groups of bushes. The front face is in the direction of the intersection of North and South Main Street, North Street, and Union Street.
The plaque is a rectangular bronze monument on a rock. It stands about three feet high. The text on the monument is as follows:
